To get a duplicate voter ID, first you need to visit the official website of the Chief Electoral Officer of your home state and download a copy of Form EPIC-002.
This form is used to issue Photo Voter ID Card.

If your voter ID card is lost or stolen, you must attach a copy of the FIR.
Also, the document should include a passport size photograph, address and identity proof.

You will be given a reference number.
With this number, you can track the status of your application by visiting the State Election Office website i.e. whether your voter ID card has been generated or not.
Once you submit your form, it is first verified, then the duplicate card process begins.
You will be notified after verification. Then you can go to the local election officer to get it.

You can apply offline for generation of a duplicate voter ID card.
For this, you have to go to the office of the election officer and apply with the necessary documents.
